Tehran, Iran. 3 Dentist. (Received 31 August, 2009 Accepted 22 June, 2010) Abstract Background and Aim : Marginal gap determines the success rate in all ceramic crown restorations. However, there is insufficient information about marginal fitness of different ceramic systems. The aim of this study was to compare the marginal gap in three different kinds of ceramic restorations namely “In-Ceram”, “IPS-e.max Press” and “IPS Empress2” . Materials and Methods : This in-vitro experimental investigation was conducted using a single blind technique. A total of 10 upper incisor teeth were prepared with size reduction 7mm in height, and 5mm diagonally. With a 1mm shoulder finishing line and tapered about 3 degrees. Then 10 ceramic crowns were built on top of each preparation. After cementation they were grinded at buccolingual direction. Finally, marginal gap analysis was conducted using Stereotactic electronic microscope (SEM). Results : The marginal gap in IPS e.max pres was 29.22 (± 8/59) μm. In–Ceram showed 49.94 (± 9/32) μm and IPS Empress 2 group demonstrated 40.88 (± 5/84) μm . Conclusion : The gap in IPS e.max Press was less than the gap in In-Ceram and IPS Empress 2 in full ceramic restorations. (Received 29 July, 2009 Accepted 29 June, 2010) Abstract Background and Aim : Although all-ceramic restorations are widely used, there is lack of information about how the fit is affected by fabrication procedures. This study evaluated the marginal gap of Pocera all-ceramic crowns before and after porcelain firing cycles . Materials and Methods : This was an experimental study, using ten standardized all-ceramic crown of Procera system that were fabricated on metal die crowns of the conventional metal-ceramic restorations served as control group. A device was used to retain specimen during measurement and to reposition them on the device after each manufacturing process. The specimens were not cemented and the gaps were measured on the metal die using stereo microscope. The crown systems were compared by using ANOVA and paired sample t-tests . Results : The conventional metal ceramic (69.17 ± 44.8 µ ) and Procera crowns (61.35 ± 43.79 µ ) demonstated nearly identical marginal discrepancy values. Statistical analyses demonstrated no significant differences in the marginal discrepancy values of Procera and metal ceramic crowns. The findings indicate that the addition of porcelain to the copings of Procera did not cause significant change in marginal discrepancy values (P=0.7), although there were a significant difference in marginal discrepancy values of control group after firing cycles (P=0.001) Conclusion: Within the limitation of this study, it was concluded that Procera all ceramic crown system demonstrated an acceptable marginal fit. The porcelain firing cycle did not affect the marginal fit of this system . Key words : Dental restoration – Allceram – Marginal adaptation – Procera - Dental porcelain . Isfahan, Iran. 2 Dentist. (Received 10 March, 2009 Accepted 29 June, 2010) Abstract Background and Aim : Bleaching may cause decrease in Calcium, Phosphate and Fluoride content of enamel. However, the topical use of fluoride can help reabsorb those minerals. The aim of this study was to determine the effect of fluoride therapy before and after bleaching on enamel dye penetration. Materials and Methods : Sixty extracted carries-free human premolars were selected. All teeth surfaces were covered by nail varnish, except a square about 6 mm² on the buccal surfaces for exposure to fluoride and bleaching agent. The teeth were randomly divided into 5 groups (n=12) and then stored in artificial saliva at 37ºC for 7 days. Each group was processed as below: Group1: Specimens with no treatment were immersed in 50% silver nitrate solution. Group2: Specimens were bleached by 45% carbamide peroxide for 30 minutes twice a day for 5 days. Group3: the sodium fluoride varnish was used on specimens for 5 minutes. Group4: After bleaching like group2, specimens were stored in artificial saliva. Fluoride therapy was performed next like group3. Group5: All processes were conducted like group4 after fluoride therapy. All specimens were cut bucco-lingually along the longitudinal axis and stored in developer solution for 6 hours. Stereotactic microscope images were captured by digital camera for dye penetration assessment. Data was analyzed using SPSS (11.5) software and ANOVA and Kruskal Wallis tests . Results : Data showed that bleaching can significantly (P=0.001) increase dye penetration. However, fluoride treatment can significantly decrease dye penetration (P=0.001). Conclusion : Fluoride therapy is highly recommended after bleaching process. Key words : Esthetics dental - Dental enamel permeability - Fluoridation . (Received 29 Nov, 2009 Accepted 30 June, 2010) Abstract Background and Aim : Eosinophils are suggested to have had participated in neoplastic invasion and metastasis. The aim of the present study was to evaluate eosinophilic infiltration in oral squamous cell carcinoma (OSCC) and assess its correlation with cervical lymph node metastasis and histological grading . Materials and Methods : This cross-sectional study used 20 OSCCs samples with lymph node metastasis and 20 samples without (NLM) were selected from the files of Tehran Cancer Institute. Hematoxylin-eosin stained sections were used to evaluate the presence of eosinophils in neoplastic tissues and OSCCs grading. Eosinophils were counted in 10 dense- and 10 classic-high-power-fields and graded from 0 to 4. Statistical analysis as well as Mann-Whitney-U and Kruskal-Wallis tests. Results : The largest percentage of cases in OSCCs with and without LNM based on Eosinophil grading was 50% in grade 1 and 70% in grade 2, based on the dense-field count and 50% in grades 1 and 0 and 70% in grade 1, when using the classic-field counting method, respectively. Eosinophilic infiltration was not significantly different between the two groups (P>0.05). A significant difference in mean Eosinophilic count was observed among the histological grades when using the dense-field method (P=0.004), but not the classic technique (P=0.029). Conclusion : Within limitations of present study it appears that the presence of Eosinophils in OSCC specimens could not be used as a clinical predictor for lymph node metastasis, however further investigation with a larger sample size is required to clarify the role of these inflammatory cells in OSCC cases. (Received 30 Nov, 2009 Accepted 30 June, 2010) Abstract Background and Aim : The use of antibiotics in conjunction with scaling and root planning is indicated in several cases of periodontitis. Azithromycin is effective against gram negative microorganisms and has a long half life. The aim of this study was to evaluate the effectiveness of Azithromycin on clinical parameters in patients with chronic periodontitis. Materials and Method : This randomized double-blind placebo-controlled clinical trial was conducted by recruiting 40 patients with chronic periodontitis referring to the Periodontics Department of Tehran University Dental School. The Papilla Bleeding index (PBI), Probing Depth (PD), and Clinical Attachment Level (CAL) were measured. The clinical examination was folloed by scaling and root planning (SRP). Patients were assigned randomly to test (Azithromycin, 500 mg once daily for 3 days) and control (placebo, once daily for 3 days) groups one month after SRP. All parameters were measured again 2 and 4 months after baseline evaluation . Results : Before administration of antibiotics (one month after baseline) the levels of clinical parameters (PBI, PD, and CAL) were not statistically different between the test and control groups. All clinical parameters (PBI, PD, and CAL) were significantly improved in both test and control groups one month after baseline (P‹0.05). In addition, all parameters were significantly lower in test group when compared with control group at 2 and 4 months (P‹0.05). Conclusions : Within the limits of this study, oral administration of Azithromycin in conjunction with scaling and root planning was useful in the treatment of moderate to severe chronic periodontitis. (Received 10 Dec, 2009 Accepted 19 July, 2010) Abstract Background and Aim: Lack of sufficient bone is a main problem in implant treatment. Bone graft materials are potential solutions to this problem. Bioose is a grafting material that is routinely used for this purpose. In some studies autogenous demineralized dentin matrix (ADDM) has been used as bone substitute. The aim of this investigation was to compare the two grafting materials (ADDM and Biooss) on vertical bone augmentation in parietal bone of rabbits . Materials and Methods: Six white Newsland rabbit were used in this study. Two screws were inserted on the parietal bone of each rabbit in anteroposterior direction. Biooss powder was put around the anterior screw and ADDM particles around the posterior screw and both were covered with gutta percha membrane. The rabbits were sacrificed after 12 weeks and the specimens were sent for histological examination. Mann Whitney and paired T-tests were used to analyze the data. Results: Range of bone height in Biooss group were between 0.1-2.1 and in ADDM group were 0.4-3.2 mm. Mean Bone height in Biooss group was 1.43± 0.84 and in ADDM group was 1.4±1.03. Mann Whitney test showed no significant difference between the two groups in bone height. Conclusion: In this study the vertical bone augmentation in Biooss and ADDM groups were the same. Therefore, ADDM was an effective biomaterial for vertical bone augmentation in this study. Key words: R abbit – V ertical bone augmentation - A utogenous demineralized dentin - Biooss . (Received 3 March, 2010 Accepted 19 July, 2010) Abstract Introduction : Hypophosphatasia is a rare inherited disorder characterized by deficient activity of tissue non-specific alkaline phosphatase (found in the bone, liver and kidney) which leads to abnormal bone and tooth mineralization. The symptoms are highly variable in their clinical manifestation, ranging from stillbirth without mineralized bone to early loss of primary teeth without bone symptoms. The diagnosis is based on low levels of serum alkaline phosphatase and increased serum or urine concentrations of phosphoethanolamine, pyridoxal 5’-phosphate and inorganic pyrophosphate. Six clinical forms of Hypophosphatasia are currently recognized: prenatal (lethal), prenatal benign, infantile, childhood, adult and odontohypophosphatasia . Case report : A 27-month-old girl was referred to the Paediatric Dentistry Department of Shiraz Dental School with chief complaint of spontaneous loosening and shedding of anterior deciduous teeth. In clinical examination, it was noticed that, teeth number 61, 62, 71, 72 and 81 were absent. There was no sign of gingival inflammation. According to the height and weight curves, growth of patient was below the normal rate. About 1 year delay in the date of her standing or walking capability was recorded in the patient’s medical history. Also there was a history of foot pain and early fatigue. CBC, serum alkaline phosphatase and urine phosphoethanolamine tests were requested for the patient. Results showed decreased serum alkaline phosphatase and increased urine phosphoethanolamine. In histological survey of exfoliated teeth, the absence of cementum on root surfaces was detected. Also, the patient’s physician reported skeletal deformities and defective mineralization in her hands and feet. Key words : Hypophosphatasia - Alkaline phosphatase - Premature teeth loss.
